NRP/531 is where nurse practitioner students learn to document like providers. Notes lose points for vague language, such as "lungs normal" instead of describing breath sounds, and for assessments that name a diagnosis without the findings that support it. A plan that addresses only the main complaint and ignores the second problem in the scenario is another frequent loss.
